Below are three very important forms for all student athletes to complete and turn in before they are eligible to play any sport at Horizon Christian High School or Middle School.

Please make sure to fill out all forms and include a parent/guardian signature where needed, plus a sports participation fee (please read form for correct amount needed) made out to HCHS/HCMS Athletics.
